What is an ROI request?

ROI stands for Release of Information. It’s a formal request to have your medical records sent to you or to another doctor.


How it works:

  1. Fill out the form

    Enter your details, the clinic’s info, and (optional) the email of the doctor who should receive your records.

  2. Sign electronically

    The form is auto-generated. You’ll sign it online and upload an ID copy to validate your request.

  3. Get a copy by email

    The request is sent to the clinic, and you receive a copy. Everything is encrypted and secure.
